1 emergency lighting was inoperative in all parts of the vessel;
2 the gyro compass heading information was not being transmitted to the bridge
3 the exhaust manifold piping on the emergency fire pump was defective; 4 the bridge first aid kit included expired items and the medical certificate for the medical stores had expired.
5 In addition the bridge pyrotechnics,(8 out 12 para red rockets) had expire.
6 the water from the showers was being used to flush the toilets as there was no other water supplying them as the sea water pump was inoperative.
