- Company verification, review and evaluation.
The company should establish procedures to ensure the
ship is maintained in accordance with the provisions
relevant to the rules and regulations established by the
company. The technical and/or operational nature of
deficiencies 02, 03, and 04 provide objective evidence of
the serious failure to implement the ISM Code. Prior to
the OCMI clearance of these deficiencies, an external
audit report must be submitted to this office.

- Fixed fire extinguishing installation
After any survey of the ship, no change shall be made in
the structural arrangements, machinery, equipment and
other items covered by the survey without the sanction of the Administration. PSCO observed unapproved
modifications to the fire main piping system within the
reefer control store which which is in direct conflict with
fire control plan.

- Other (fire safety)
No flammable or combustible liquids may be stowed in
any accommodation, control, or service space (other than a paint locker). PSCO observed flammable and
combustible liquids stored in the reefer control store,
which is not designated as a paint locker on the fire
control plan
